INVOICE, CUSTOMER SERVICE AGREEMENT & LIABILITY WAIVER
(Master Agreement – Nationwide Franchise Use)

This Customer Invoice & Service Agreement (“Agreement”) sets forth the terms and conditions governing all services, parties, appointments, and events provided by any authorized Little Princess Spa® franchised location.
ENTERTAINMENT PURPOSES ONLY All services provided by Little Princess Spa® are for entertainment purposes only and are not professional cosmetology, salon, medical, or therapeutic services.
INCORPORATION OF MASTER WAIVER ADDENDUM This Agreement incorporates by reference the Little Princess Spa® Master Waiver Addendum (“Addendum”). The Addendum supplements and forms part of this Agreement. By signing or electronically accepting this Agreement, the undersigned also agrees to be bound by the Addendum.In the event of any conflict between this Agreement and the Addendum, the Addendum shall control to the fullest extent permitted by applicable law.

ACKNOWLEDGMENT & ACCEPTANCE By booking, purchasing, attending, or participating in any party, appointment, or event at Little Princess Spa®, whether online, in person, by phone, or as a guest, you acknowledge that you have read, understood, and agreed to this Agreement and the incorporated Addendum on behalf of yourself and any minor(s) in your care.
BRAND-WIDE USE & STANDARDIZATION This Agreement is approved for use by all authorized Little Princess Spa® franchised locations . No franchisee, employee, contractor, or representative may modify, remove, or add language to this Agreement without prior written approval from Little Princess Spa® Corporate . This provision ensures consistency, legal compliance, and brand protection across all locations.
GOVERNING LAW & VENUE This Agreement shall be governed by and construed in accordance with the laws of the state in which the Little Princess Spa® location providing the services is physically located. Any legal action or proceeding shall be brought exclusively in the courts located within that state and county, to the extent not required to be resolved by arbitration under the Master Waiver Addendum.
PAYMENTS, DEPOSITS & NO-REFUND POLICY
All deposits, payments, tickets, and fees paid to Little Princess Spa® are NON-REFUNDABLE AND NON-TRANSFERABLE . Deposits secure staffing, scheduling, and preparation and are required to reserve any party, appointment, or event. Any remaining balance, applicable gratuity, or authorized charges are due in person on the date of service , unless otherwise stated in writing by the franchised location. Gratuities are not included in package pricing. A 20% gratuity is recommended for staff unless otherwise disclosed. Failure to appear, late arrival, early departure, or refusal of services does not entitle the customer to a refund, credit, or rescheduling. Payment methods, timing, and whether final balances must be paid in person may vary by location and will be disclosed on the location’s invoice, booking confirmation, or written payment instructions.
SERVICE-SPECIFIC TERMS
*DAY SPA APPOINTMENTS
Thank you for booking your Princess’ appointment at Little Princess Spa®. A 50% deposit is required to book a day spa appointment and any hair salon services, which is non-refundable and non-transferable. The credit card used for the deposit and remaining balance must be under the name of the person who booked the appointment. Photo identification and the credit card used will be verified on the day of the appointment. Appointments are scheduled for a specific date and time and cannot be changed once booked. Failure to show up for an appointment will result in forfeiture of the deposit. If you arrive more than 15 minutes late , the appointment will be automatically canceled and the deposit will be forfeited. A new appointment and deposit will be required.If a child attends a scheduled appointment and chooses not to receive services, the full package amount remains due . Day Spa Appointments may not bring food, drinks, cake, cupcakes, party favors, or decorations into the spa. Runway dress-up is not included for Day Spa Appointments unless the selected Day Spa package specifically includes a change of clothes or the LPS Princess Set (tutu, LPS shirt, wings, wand, and tiara) is purchased. A maximum of two adults per child is permitted. Gratuities are not included. A 20% tip of the total appointment package is recommended for staff.
*EVENTS
Thank you for booking your Princess’ event at Little Princess Spa®. If a child attends a scheduled event and chooses not to receive services, tickets are not refundable . A maximum of one adult per child is permitted at events. The credit card used to purchase event tickets must be under the name of the purchaser and will be verified with photo identification on the day of the event. Event tickets are valid only for the specific date, time, and location purchased. Events begin and end at their scheduled times. Late arrival may result in reduced services without compensation. Failure to attend a scheduled event will result in forfeiture of the ticket. Tickets may not be transferred, rescheduled, or applied to another event. Little Princess Spa® reserves the right to cancel or reschedule events when necessary. Gratuities are not included. Guests may leave a cash tip for staff at the event.
*PARTIES
Thank you for booking your Princess’ birthday party at Little Princess Spa®. The spa is a private venue reserved for your celebration. A 50% deposit is required to secure the party date and time, which is non-refundable and non-transferable. The remaining balance and applicable gratuity are due at the end of the party. Gratuities are not included; a 20% tip is recommended for staff. Depending on the selected party package and the number of confirmed guests, an additional hour may be required. Any additional hour must be scheduled at the time of booking and cannot be added later. The credit card used for the deposit and balance must be under the name of the person who booked the party and will be verified with photo identification on the day of the party. Party dates and times may not be changed except in the case of an emergency supported by a 72-hour written doctor’s notice . Once a party package is booked, it may not be downgraded; however, upgrades are permitted. Little Princess Spa® staff will contact the host during the week of the party to confirm the final number of children attending. The number confirmed is the number billed, regardless of attendance . Additional guests beyond the confirmed number may not be accommodated due to time and staffing limitations. Late arrivals may result in reduced services. If the party starts 15 minutes or more late for any reason, Little Princess Spa® reserves the right to modify or reduce services to stay within the scheduled time. Pull-string piñatas are permitted. Stick piñatas are not allowed. If a piñata is included, only one additional game may be played at the end of the party. Pink lemonade cheers occur within the first 10–15 minutes of the party. Guests arriving after this time will not participate. Guests may or may not eat together depending on time constraints; cake time is shared together. Children may choose not to participate in some or all services. If a child attends a scheduled party and chooses not to receive services, the full amount remains due . The spa has an open floor plan. Dressing rooms do not have doors, allowing parents and guardians to maintain visual supervision at all times. Services are not re-done. Guests who decline services initially cannot be accommodated later. Cake cutting is handled by staff. If you wish to photograph the cake prior to cutting or have special traditions, please notify staff in advance. Children are encouraged to arrive with nails clipped and polish-free. Gel polish is not removed. Guests with gel polish will not receive mini manicure or pedicure services. If a service included in a package is not permitted for the group, it may be substituted with glitter tattoos. For hairstyling services, children should arrive with clean, brushed, and fully detangled hair . Little Princess Spa® staff do not provide detangling services, and heavily tangled hair may limit or prevent certain hairstyles from being performed within the allotted party time. Parties begin and end strictly on schedule. Guests may arrive no more than 30 minutes early and may not remain after the party end time. An additional $250 fee will apply if the party exceeds the scheduled end time. Food, cake, cupcakes, and party favors are permitted. Ice cream cakes or ice cream are not allowed due to refrigeration limitations. Party utensils must be provided by the host unless included in the package; otherwise, a $45 supply fee may apply. Loose balloons and table decorations are permitted. Balloon arches, columns, confetti, bubbles, wall panels, backdrops, ceiling decorations, tape, nails, pins, or anything that may damage the space are not permitted. Furniture may not be moved. Outside vendors are not permitted. Personal character companies may not be used. Wine and champagne for adults are permitted. Ice must be provided by the host.
DROP-OFF PARTIES, DAY SPA APPOINTMENTS & EVENTS
Parents, legal guardians, and caregivers are responsible for the arrival, supervision, and pickup of all children attending parties, appointments, or events. Little Princess Spa® staff are not responsible for monitoring who children arrive with or who they depart with. Hosts and caregivers are responsible for coordinating pick-ups and ensuring children are dismissed safely at the end of the scheduled time. Children from previous parties may not remain unattended while waiting for pickup. Safety cameras do not substitute adult supervision.
NO CUSTODIAL CARE / NOT A CHILDCARE FACILITY
Little Princess Spa® provides entertainment services and themed experiences for children and families. The spa is not a daycare, childcare provider, school, or custodial supervision service. At all times, parents, legal guardians, or designated caregivers remain fully responsible for the supervision, conduct, safety, and well-being of any minor in their care while on the premises or participating in services. Staff members facilitate entertainment activities but do not provide continuous one-to-one supervision or custodial care.

Little Princess Spa® staff are not responsible for verifying custody, monitoring pickup arrangements, or supervising children outside the scheduled service experience.
SUPERVISION & ASSUMPTION OF RISK
In addition to the drop-off and pickup responsibilities above, parents, legal guardians, or authorized caregivers are solely responsible for the supervision, care, and conduct of any minor in their care at all times. Participation in services and activities involves inherent risks. By entering the premises, the undersigned voluntarily assumes all such risks on behalf of themselves and any minor(s) in their care, as further detailed in the Master Waiver Addendum.
EXAMPLES OF POTENTIAL RISKS
Participation in spa activities, parties, events, and general use of the premises may involve risks that cannot be completely eliminated. These risks may include, but are not limited to:
Slips, trips, or falls on floors, rugs, steps, furniture, or equipment • Minor burns from warm water, lighting equipment, or decorative elements • Allergic reactions or skin sensitivities to cosmetics, nail polish, glitter, makeup, lotions, or similar products • Choking hazards from food, candy, small craft items, beads, or decorative materials • Staining of skin, hair, clothing, or personal items from cosmetic products • Minor injuries occurring during dress-up activities, games, runway walks, or movement within the venue • Contact with decorative elements, furniture, props, or party equipment • Incidental contact between children during activities
These examples are illustrative only and are not intended to represent a complete list of potential risks. By entering the premises and participating in services, the undersigned acknowledges that participation is voluntary and accepts these inherent risks.
THIRD-PARTY VENDORS
Third-party vendors may include, but are not limited to, characters, DJs, photographers, limousine services, face painters, decorators, and other add-on services offered in connection with a party, event, or appointment. Selection or purchase of a third-party vendor online does not constitute confirmation. All vendor services are subject to availability and are considered confirmed only after direct confirmation by Little Princess Spa® . All third-party vendors are independent contractors and are not employees, agents, or representatives of Little Princess Spa®. Little Princess Spa® is not responsible for the acts, omissions, performance, conduct, or availability of any third-party vendor. Vendor services may be modified, substituted, or canceled due to unforeseen circumstances, including emergencies affecting the vendor. If a third-party vendor is unable to perform, the vendor-related charge will be removed from the invoice. This shall be the sole remedy available , and no additional refunds, credits, or compensation will be provided.
LIMOUSINE RESERVATIONS
A non-refundable deposit is required to secure the date and time for limousine services. If limousine services are canceled for any reason, the deposit is non-refundable and non-transferable and may not be applied as a credit toward any other service, party, or event.
GIFT CERTIFICATES
Gift certificates are non-refundable, may not be redeemed for cash, may not be used to purchase other gift certificates, and are valid for one (1) year from purchase unless otherwise stated. Gift certificates must be used by calling to book an appointment, party, event, or ticketed experience. If a gift certificate is used toward a deposit or booking, all terms of this Agreement and the Master Waiver Addendum apply .
BOUTIQUE PURCHASES
All boutique items are final sale and are non-refundable and non-exchangeable.
PRODUCTS, SERVICES & COSMETOLOGY DISCLOSURE
Little Princess Spa® services are not performed by licensed cosmetologists and are provided for entertainment purposes only . The spa does not use powered equipment, cuticle tools, or harsh chemicals during mini manicure or mini pedicure services. Products used are selected with children in mind; however, reactions may still occur. Parents, guardians, or caregivers are responsible for informing staff of any allergies or sensitivities prior to services. Certain products used during services or included in party favors (including nail polish, makeup, glitter, face paint, hair color, or similar items) may stain skin, hair, clothing, or nails. Removal recommendations may be provided upon request.
HYGIENE & PRE-EXISTING CONDITIONS
Little Princess Spa® maintains reasonable sanitation and hygiene practices for all services and equipment. However, because services involve interaction among guests and children, the spa cannot guarantee the prevention of naturally occurring conditions that may be transmitted in public environments.
Such conditions may include, but are not limited to:
Little Princess Spa® is not responsible for diagnosing, treating, or preventing any medical or dermatological condition. If a child has a known skin condition, allergy, or sensitivity, it is the responsibility of the parent, guardian, or caregiver to notify staff prior to services. To the fullest extent permitted by law, Little Princess Spa® shall not be liable for conditions that may arise from participation in activities or interaction with other guests.
COMMUNICABLE DISEASE RISK & RELEASE
Participation in parties, appointments, and events at Little Princess Spa® may involve interaction with other guests, children, and staff within an indoor environment. As with any public venue, exposure to communicable illnesses, viruses, or contagious conditions may occur.
By entering the premises or participating in services, the undersigned acknowledges and accepts the inherent risk of exposure to communicable diseases, including but not limited to influenza, COVID-19, common colds, or other contagious conditions.
To the fullest extent permitted by law, the undersigned releases and agrees to hold harmless Little Princess Spa®, its franchisor, franchisees, owners, officers, employees, contractors, and representatives from any claims, damages, liability, or expenses arising from or related to exposure to or transmission of any communicable disease before, during, or after participation in services or presence on the premises.
Guests who are experiencing symptoms of illness, fever, or known contagious conditions are encouraged not to attend or participate.
SECURITY, CONDUCT & PREMISES RULES
Stealing from the spa or boutique, fraudulent credit card use, or use of counterfeit currency will result in prosecution to the fullest extent permitted by law.
For safety, security, and quality assurance purposes, video and/or audio recording systems may be in use throughout the premises. Recordings are used internally and are not released upon request. Entry into the premises constitutes consent to such recording to the fullest extent permitted by law.
GUEST RESPONSIBILITY & PROPERTY DAMAGE
Guests are expected to behave in a safe and respectful manner while on the premises.
The parent, legal guardian, or caregiver booking or supervising the event accepts responsibility for the conduct of the children and guests attending under their reservation.
The undersigned agrees to be financially responsible for any damage to the facility, furnishings, equipment, decorations, or property caused by themselves, their child(ren), or any guest attending under their reservation, beyond normal wear and tear.
This may include, but is not limited to:
Damage to furniture, décor, walls, fixtures, or equipment • Staining, breakage, or destruction of spa property • Damage caused by prohibited decorations or materials • Damage resulting from guest misconduct or unsafe behavior
To the fullest extent permitted by law, the undersigned agrees to indemnify and hold harmless Little Princess Spa®, its franchisor, franchisees, owners, employees, contractors, and representatives from any claims, damages, liabilities, or expenses arising from damage caused by the undersigned, their child(ren), or their guests.
Little Princess Spa® reserves the right to charge the payment method on file for reasonable repair or replacement costs associated with such damages where permitted by applicable law.
PHOTO & MEDIA RELEASE (MINORS)
By attending or participating in any party, appointment, or event, the parent, legal guardian, or caregiver grants Little Princess Spa® permission to use photographs or video taken during the experience for lawful purposes, including marketing, advertising, social media, and website content. No compensation, royalties, or fees shall be owed for such use. This includes use on Little Princess Spa® websites, social media accounts, digital ads, printed materials, and other marketing channels.
CLOTHING & COSTUME POLICY
Little Princess Spa® staff do not change, remove, or assist with changing clothing for any child. If a costume, accessory, or themed attire is included in a package or brought by a guest, any clothing change must be completed by the child’s parent, legal guardian, or caregiver, or by the child independently under parental supervision. Staff may distribute costumes or accessories but are not responsible for dressing, undressing, or supervising clothing changes.
COMMUNICATION CONSENT
By providing contact information, the undersigned consents to receive calls, emails, and text messages from Little Princess Spa® locations regarding bookings, updates, promotions, and related communications. Consent may be revoked in accordance with applicable law.
PAYMENT AUTHORIZATION & CHARGEBACK POLICY
The undersigned authorizes Little Princess Spa® to charge the credit card or payment method on file for agreed-upon purchases, remaining balances, authorized gratuities, or unpaid amounts due. The undersigned confirms that the payment method used belongs to them and that photo identification may be required to verify payment. The undersigned agrees not to initiate chargebacks or payment disputes for authorized charges or services reserved or provided under this Agreement, except where otherwise required by applicable law.
ELECTRONIC SIGNATURE & ACCEPTANCE
By electronically signing, clicking acceptance, checking a box, or submitting payment online, the undersigned agrees that such action constitutes a legally binding signature with the same force and effect as a handwritten signature.
SURVIVAL & SEVERABILITY All payment obligations, liability waivers, releases, indemnifications, and dispute-resolution provisions shall survive the completion of any party, appointment, or event. If any provision of this Agreement is held invalid or unenforceable, the remaining provisions shall continue in full force and effect.
FORCE MAJEURE
Little Princess Spa® shall not be liable for cancellation, delay, modification, or interruption of services caused by events beyond its control including but not limited to weather, power outages, emergencies, acts of God, government orders, pandemics, or facility issues. Services may be rescheduled when possible but refunds are not guaranteed except where required by law.
CONTINUING EFFECT
By signing or electronically accepting this Agreement, the undersigned agrees that it applies to all past, present, and future visits to any Little Princess Spa® location unless revoked in writing prior to services being rendered.
