REQUIRED: This waiver must be completed for all participants.

Welcome to Little Princess Spa!
We are delighted to have your child participate in our magical After School Program. Please read this waiver carefully. Whether this waiver is signed physically or not, your presence at or use of our facility constitutes implied agreement to the full contents of this document.

Program Participation Consent

I, the undersigned parent or legal guardian, hereby give permission for my child to participate in the Little Princess Spa® After School Program. I understand that the program includes enrichment activities, light spa-themed play, arts & crafts, themed games, and supervised free time. I understand this is a recreational and social program and is not a school or academic tutoring service.

Assumption of Risk & Liability Waiver

Participation in our after school program includes a variety of spa-themed and recreational activities, including hair-dos, mini-mani, mini-pedi, facials, crafts, games, and more. These activities are for entertainment only and are not performed by licensed cosmetologists. We do not use powered tools or harsh chemicals, but minor reactions may still occur.

By enrolling your child:

  • You accept all risks, known or unknown, including injury, allergic reactions, or accidents.
  • You release Little Princess Spa, its owners, staff, and affiliates from any liability.
  • You assume responsibility for informing staff of any allergies or special needs.

We strive for a safe, joyful environment. Our staff is trained and attentive, but parents/guardians are responsible for any child they register.

Medical Release & Authorization

In the event of an emergency, I authorize Little Princess Spa® to seek medical attention for my child. I understand that:

  • Reasonable efforts will be made to contact me or an emergency contact.
  • I am financially responsible for any medical care needed.

Emergency Medical Financial Responsibility:

I understand that I am solely responsible for all costs related to any emergency medical treatment my child may require, including ambulance transport or urgent care services, whether or not I am immediately reachable at the time of the incident.

Behavior & Emotional Support Policy

Children must:

  • Follow rules and staff instructions.
  • Behave safely and respectfully.
  • Be fully potty trained (no diapers).

We follow a behavior management protocol:

  1. Verbal warning
  2. Parent meeting
  3. Possible dismissal

I understand that while children are given gentle redirection and positive support, three behavior warnings may result in dismissal from the program with no refund.

No refunds will be given for dismissal due to behavioral concerns.

We welcome neurodiverse children. If your child requires additional support (e.g., for ADHD, Autism), please notify us in advance. In cases where needs exceed our capacity, we may request early pickup or withdrawal.

Homework Supervision Disclaimer

As part of our After School Program structure, Little Princess Spa® offers supervised homework time as a quiet period for children to work independently on their school assignments.

Please note:

  • We are not a tutoring center or academic support provider.

  • Our staff will supervise only and cannot assist with or ensure the completion or accuracy of any homework.

  • If a child chooses not to work on their homework or does not complete it during the program, Little Princess Spa® is not responsible.

We encourage parents to review homework at home and communicate directly with their child’s school for academic support needs.

Health & Hygiene

By signing, you confirm that your child:

  • Is in good health and able to participate.
  • Has had no recent contagious illness or exposure.
  • Will arrive clean, appropriately dressed, with snacks, water, and a change of clothes.

Outfit Changes: In case of accidents, we will contact the parent. Staff will not assist with changing; a guardian must provide clean clothing..

Attendance & Pickup Policy

If your child is absent without notice, we will attempt to contact you to confirm their status. If your child is not picked up by the scheduled end time, late pickup fees apply at $5 per minute.

Chronic lateness or unreported absences may result in dismissal from the program.

Emergency Pickup Policy:

If my child experiences a medical or behavioral emergency and I or my authorized contacts cannot be reached within one hour of notification, I understand that Little Princess Spa® staff may contact emergency services or appropriate child welfare authorities to ensure the child’s safety.

Transportation Release (If Applicable)

If your child will be transported to Little Princess Spa® from school or by a third party (e.g., school bus, carpool, rideshare), you agree to release Little Princess Spa® and its staff from any liability related to transportation. You are responsible for ensuring the child’s safe arrival and proper authorization for school release.

Emotional Regulation Disclaimer

Our team is trained to support children in a nurturing environment, but we are not a therapeutic or behavioral health program.
If your child exhibits aggressive behavior, repeated emotional outbursts, or distress that prevents them from participating safely, we reserve the right to request early pickup or program withdrawal.

Registration Fee: $49 (one-time, non-refundable)
Late Pickup Fee: $10 for every 10 minutes after 5:30 PM

Drop-In Policy:

  • Drop-in sessions must be booked at least 24 hours in advance

  • Availability is not guaranteed

  • No-shows will be charged

Billing & Payment Terms

  • Auto-Billing Authorization: By signing this form, you authorize Little Princess Spa® to charge your payment method on file automatically on the 1st of each month.
  • Prorated Start: New enrollments mid-month will be billed at a prorated rate.
  • Late Payments: A $25 late fee applies to payments not received by the 5th of the month.
  • Declined Payments: May result in additional fees and immediate program suspension.
  • No Refunds: Tuition is non-refundable and non-transferable. No credit is given for missed days unless approved by management.

Cancellation & Withdrawal Policy

  • Notice Deadline: Written cancellation must be submitted at least 7 days before the next billing cycle.
  • How to Cancel: Submit written notice via email or in person using the official cancellation form.
  • Failure to Notify: Families failing to cancel in time will be billed for the next month.
  • Behavior-Related Dismissal: No refunds will be issued for dismissal due to behavioral issues.

Program Environment & Limitations

  • All staff are female and trained in general supervision.
  • No electronics are allowed unless authorized.
  • Surveillance cameras are used for internal purposes.
  • We are not responsible for lost or damaged items.
  • Injuries resulting from failure to follow rules are not our liability.

COVID-19 & Contagious Illness Release

You affirm that:

  • Your child or household has had no COVID-19 or flu symptoms in the last 14 days.
  • You release Little Princess Spa® from liability related to any virus exposure.

Media & Photo Consent

Social Media & Photography

  • I give permission for my child to be photographed or recorded during program activities.
  • I understand that content may be used by Little Princess Spa® for social media or marketing.
  • Parents may not use photos or videos taken during our programs for commercial purposes or in any way that defames or misrepresents the brand.
  • Little Princess Spa® corporate and franchise locations reserve the right to request the removal of inappropriate or misleading online content.

Franchise & Brand Protection

I acknowledge that Little Princess Spa® is a registered trademark. I agree not to replicate, copy, imitate, or attempt to create a similar business concept based on my experience with the program, in whole or in part.

Indemnification Clause

I agree to indemnify and hold harmless Little Princess Spa®, its franchisees, employees, owners, agents, and contractors from any and all claims, damages, or liabilities arising from participation in the program.

Severability & Binding Arbitration

Any dispute under this agreement shall be resolved by binding arbitration in the state where the spa is located. If any provision is found unenforceable, the remainder of this document shall remain valid.

Safety & Supervision

What are your staff-to-child ratios?

At least 3 staff members are assigned to every 20 children.

Are staff background-checked and CPR/First Aid certified?

All staff are background-checked. While not all are certified, a CPR/First Aid trained supervisor is on site at all times.

What emergency procedures are in place?

We follow detailed emergency protocols for evacuation, illness, and injury. Parents are contacted immediately in case of emergency.

How do you handle injuries, illness, or allergies?

Minor injuries are treated with first aid and documented. For illness or allergies, parents will be contacted immediately.

Is there secure check-in/check-out?

Yes. Doors are locked during program hours, and ID is required for pickup. Only approved adults on the Authorized Pickup Form may sign out a child.

What are the program hours?

The program runs from 2:30 PM – 5:30 PM on designated weekdays.

Is early drop-off or late pickup available?

No early drop-off is allowed. Late pickup is subject to late fees.

What does a typical day look like?

Children enjoy spa services, homework support, crafts, games, snack time, and themed activities.

Are there daily themes or routines?

Yes! Each week has rotating fun and educational enrichment themes.

What types of activities are offered?

Spa services, crafts, creative games, light physical activity, and occasional themed events.

Is it mostly indoors, outdoors, or a mix?

All activities take place indoors.

Are there field trips or special events?

No field trips, but themed days and surprise activities may occur.

Is screen time allowed?

Personal devices are not permitted. Limited supervised screen time may be used occasionally for group activities.

Do you provide snacks or meals?

No. Please pack a nut-free snack and water bottle daily.

Are you nut-free or allergy-friendly?

Yes. We are a nut-free facility. Do not send peanut or tree nut products.

Is there a fridge to store snacks?

No. Please send snacks in an insulated bag with an ice pack if needed.

What age groups do you accept?

We welcome children ages 5–12.

How are children grouped?

Generally by age, but mixed activities are included.

Does my child need to be potty trained?

Yes. All participants must be fully potty trained.

We enforce a zero-tolerance policy for bullying. Positive behavior is encouraged and supported.

Auto-Billing Authorization:

Auto-Billing Authorization: I authorize Little Princess Spa® to automatically charge my selected payment method on file for tuition according to the agreed billing cycle. I understand that a $25 late fee will be applied to any unpaid balances after the 5th of the month and that declined payments may result in additional charges and temporary suspension from the program.
